Dick Wilgus has the records from the late Bud Columbia - last Executive Director of SCIP, which contain the diver certification records of SCIP. The $10 cost  for the replacement card is to cover printing and postage.

Information Needed:

1. Diver's name on card.

2. Original instructor's name.

3. When certified.

4. Any other information you think helpful in locating your records.

5. Your current mailing address

 

Cards will be marked on back as a replacement card and date issued.
